Even small amounts of activity can make a significant difference to overall health and wellbeing.

We hope that the following resources, will help you when supporting adults living with a long term health condition to get active, in a safe way, that supports their condition.

We are Undefeatable

The We Are Undefeatable campaign is helping people with health conditions find ways to get active, whatever that looks like. There are over 15 million people in England who live with long term health conditions and there are many ways to get moving.

Here is a downloadable resource from the national campaign that you can use to remind yourself to do your check in, while also working out the reasons you want to be active. Download your ‘my daily undefeatable’ sheet here – https://weareundefeatable.co.uk/data/website-media/downloads/undefeatable_planner.pdf
